2011 Hurricane Season Boarding Checklist
If you plan to board with us this hurricane season, these items must be brought to Welleby along with your pet.
1 week's worth of pet food, especially prescription diets.
Medications, i.e.: insulin, heart & thryroid prescriptions filled and labeled.
1 week's worth of water supply, per pet. 1 gallon per day, labeled with your pet's name.
Your pet must be on flea and tick preventative.
Emergency contact information, other than owner, such as a family member or friend, authorized to pick up your pet after the storm. This person must be listed on your pet's boarding information sheet at the time of drop-off. We'll need the person's name, phone number, e-mail address, and address. They must present a photo I.D. for your pet to be released to them.
Emergency contact information for at least 2 out of the area or state, family members or friend. Phone, e-mail address, and address are required.
Signed consent form for emergency services provided by a veterinarian and or technician if needed.
Please leave us a detailed list of instructions for feeding and administering medications.
Welleby requires a non-refundable 3 day boarding fee deposit for holding a reservation per pet, price based on kennel size needed.
Your pet must be current on all vaccines. If you're not a Welleby client, you must bring or fax updated documents and current veterinary information before boarding your pet.
Note: All items left at Welleby Veterinary Center, such as water, food, medications, toys, blankets and bedding, collars and leashes, must be labeled with your pet's name.
